It’s the rule: you must be checked out by a doctor before playing sports in the Fort Worth ISD.
So, the Fort Worth ISD and John Peter Smith Hospital are teaming up to make the requirement a little easier -- and free.
Any current 6th through 11th grade Fort Worth ISD student interested in playing sports during the 2012-2013 school year is eligible to receive a free comprehensive medical examination.
Middle school athletic physicals will be given on Saturday, April 14, 2012. All exams will be conducted at the Wilkerson-Greines Activity Center, starting at 8:00 a.m.
Transportation will be provided from the Fort Worth ISD middle schools that participate in UIL athletics on the day of the exams. A bus schedule will be posted on the District Web site prior to exam day.
Because of privacy concerns and space, drop-offs and pick-ups at the examination site will not be allowed. All students must ride the bus and bring the physical exam form with them.
Any student who wishes to participate should first pick up an official UIL physical form from their campus athletic coordinator. The UIL Pre-participation Evaluation/Medical History form must be filled out and signed by a parent/guardian or the physical exam cannot be performed. Any questions answered as “Yes” on the Medical History must be explained. (The actual pre-participation evaluation and physical examination form will be filled out by the physician during the exam. Please do not bend or fold this document.)
Students should wear athletic gear such as shorts, t-shirts and athletic shoes and, if applicable, glasses or contact lenses. Female students should wear a sports bra or tank top beneath their t-shirt. Do not wear caps, jewelry, flip-flops or sandals. Students: do not bring cell phones.
